You can follow the guide I have in the guides section on how to replace the digitizer. Once you have the digitizer removed, you can proceed to replacing the home button and home flex. I suggest using a heat gun or a hair dryer to soften the adhesive holding the digitizer in place. Use a PLASTIC pry tool to remove the digitizer from the mid-frame and be gentle. You do not want to break or damage the digitizer while removing it. If you feel tension on the digitizer, simply heat it a little longer until it is easier to remove.

Putting the new home button and flex in is very simple. Just remove the old button and flex, and apply the new one. Remove all of the old adhesive from the midframe, apply new adhesive strips, and re-apply the digitizer. Make sure the home button can be pressed and there is movement. Then put the display back together and re-apply to the phone.
