You will need to do a restore and re-unlock/jailbreak. Be sure to set the phone up as NEW in iTunes after you restore.

After restoring, iTunes will pop up a window asking if you want to use OLD settings that you had before restoring. In this step, un-check the old settings (maybe with your previous iPhone name), and CHECK "set as new phone" (I don't remember exactly the words it shows).

why? I am not sure. But I read posts that setting as a new phone could reduce the possibility of getting minor problems after restoring.

If you don't set up as new it will cause issues with a phone that has been previously jailbroken. It can write "ghost" images back to the phone and trick installer into thinking programs are installed that aren't. It can also re-write the bad code that caused your spinning wheel of death so it happens all over again. Additonally.. if the phone was previously jailbroken with a different method altogether such as Dev Team soft upgrade then this creates even deeper issues with unlock/jailbroken code conflicts.
