Default upgrade 1.0.2 to 1.1.4 -- safest way
What is the safest way to upgrade my 1.0.2 to 1.1.4?

The thing is that I'm not sure what method is used to unlock my iphone. I
have an "unlock" app (also titled 'unlock') on my springboard. When I start
that, it says "All files found. Ready to go. Press Start"

I hope this helps you determine what kind of unlock tool was used on my
iphone. I can provide a screenshot of it, if you want.

Some additional information that might also help:
    Version: 1.0.2 (1c28)
    Modem: 03.14.08_G

I read numerous forums that you need to 'revirginize' the iphone first
    before you upgrade to 1.1.4.

I can't really upgrade to 1.1.4 by 'revirginizing' because my installer app
keeps crashing after an update I did weeks ago. While refreshing sources, it
just bounces back to the springboard

I am on the same boat. I the very first gen out of box 1.02 iphone that was unlocked using AnySim 1.02. I believe all iphone 1.02 were unlocked using some variant of AnySim (iUnlock whatever). I am waiting to be upgrading to 2.0 once a stabilize version for windows is out (either ILiberty or Ziphone). I've done some research already, and I do think that you'll need to re-virginize the phone, because that fixes the baseband. Then you can flash with 1.1.4 firmware in DFU recovery mode. Then use iLiberty to unlock it again.

I also have the same problem of installer not lanching after updating. I believe that you can use ILiberty to install the Installer again on your 1.02 unlocked iPhone, that way, you can run the "revirginizer". Although I am not 100% positive on this, because I never tried it. But see this thread below on how to go from a 1.02 unlocked straight to 1.1.4, it should help.
