How can you create a theme for iPhone 4S on 5.1.1
I have the theme I want to use from Cydia, however I am not sure why the icons are no longer working, but the sounds, special effects, lock screen, and backgrounds do.
Is there some special place you need to ssh themes to in iOS 5.1.1?

The theme may not have been ported for iOS 5.x.x as icon names and some locations have changed. There is a tweak in Cydia called "ThemeIconMaker" TIM, that may help you skin icons.

That app is weird? im very confused with it :/ it just doesn't seem like the app is working in any way.

In order for TIM to work, there has to be an app icon mask, overlay and perhaps shadow also for all un-skinned icons and it's location would be "Your Theme\Bundles\com.apple.mobileicons.framework".

No, these images were made for the theme that I use. You will have to make your own for your theme if it is not included. Perhaps emailing the theme creator.
You can use an icons folder, you would have to name each icon exactly as Apple has, put them all in a folder in your theme creating a new folder called "icons", turn on "SummerBoard" mode in WinterBoard and then the images will work.
