21 is recovery error not dfu.Put it into dfu mode properly.If you on 10.5.6,use automator.Are you still getting error like 16xx on dfu mode,then try to use usb hub.If it does not solve
Delete device support file&kill itunes helper
Create a new custom fw
Put it into dfu mode
If you get an error,close itunes&re-open it
Restore

This is exactly what has happened to my phone today. I followed pwnage on my mac got successful and did dfu UR In message then went to itunes and got crushed with eror 21 and again on the 16XX,so I can recieve calls and make calls with the itunes logo and usb and slide emrgency and was wondering if a att activated sim card would unlock the phone to allow icons and the phone go to normal ability to still use my Tmobile sim?

try restoring on another computer, to factory settings.
then go back to yours and try pwnage again. also if you're getting error messages drag pwnage to applications and make sure you quit before custom restore.
also its better to download the ispw file in firefox rather than safari if you're on a mac.

i was getting those for ages, then i went to another computer and restored it to factory settings through itunes.
then i went back to my computer and ran pwnage after dragging it to applications folder. make sure after its finished making the firmware, that you quit. also, if you're on a mac its best to download the ispw file on firefox rather than safari. if you are on a new mac, use a usb hub when setting iphone into DFU mode.
