I have my iphone 3G 4.2.1 w/ 06.15.00 BB installed the latest ultrasn0w from cydia and it will not unlock it just says no service.
(this phone was given to me by a friend)
I believe it was intially hacktivated and SAMS PREF was installed (ive removed that)
i obtained an original carrier sim and used redsn0w to DEACTIVATE it and let itunes RE-ACTIVATE it w/ original carrier sim and then i reinstalled ultrasn0w and im still getting the NO SERVICE error.
SAM is used to activate and allow access to the Springboard. It does not fix a 'no service' issue.
Try re-installing Ultrasn0w.
If that doesn't help and you have recently restored to 4.2.1, try re-installing the iPad BB 6.15.
Download and run redsn0w 0.9.6rc16, check one (1) option, "Install iPad baseband".
Reboot and test.
The process is fairly quick and does not effect the current iOS or jailbreak.
If it doesn't help, I would start completely over with a fresh 4.2.1 restore.
Install Cydia
Install custom bundle
verbose boot
custom boot logo
custom recovery logo
enable multitasking
enable homescreen wallpaper
enable battery percentage
deactivate
just boot tethered right now
just enter pwnd DFU mode right now
I dont see any install ipad baseband option... i know ive seen it before on older redsn0w versions or is it after this screen i can select it?
or am i missing something here?

how do i create one of these for my iphone 3g 4.2.1 ? im running windows... i know there was a mac utility that would allow me to do that ages ago but that does me no good
snowbreeze is what you will have to use on windows. A quick google of "iPhone 4.2.1 snowbreeze custom" should point you to the right version that you need.
Ok there is a fix for the no service problem. If you look on the Internet hard enough you'll find it. It's not a software problem but a hardware problem.
The problem is the inductors on motherboard has broken off. On a 3G it's on the back of the motherboard at the bottom beside the copper/brass antenna.
On a 3GS it's on the front in the same stop.
The solder is **** on the motherboards and grows brittle after awhile and the inductor falls off. It's only about 1mm big but if it's not there no signal.
This is what you need to do.
Open phone remove mother board and get a magnifying glass and solder the 2 points together. You dont need the inductor it does nothing really but make a connection from point a to point b the antenna.
A simple solder connection will work. Has for me. Ive fixed **** loads of iPhone 3G and 3gs's this way with the no signal problem.
