I believe Apple USA will unlock any *new* iPhone purchase upon checkout. It will not do unlocks for already purchased devices. The only official unlocks I have seen done so are those in the UK with carriers such as O2. I am not 100% sure on finding out the origin of the device, however you can call Apple and give the IMEI of the device, and I am sure someone can tell you where the device is from. I think you can even do so yourself online by punching in the IMEI but I can't for the life of me remember what that site was. I know it was directly through Apple somehow, but I don't know if I can't find it now bc I don't remember how I searched it or that option no longer exists. I think the fastest way is to just call Apple and ask.
Edit: just found an old thread that asked the same question. Here's a link provided by iYeow's findings that may help you, you'll have to go through that long list though.
