Inserting SIM causes iPhone to freeze
I sold a unlocked and jailbroken iPhone 3G to a buyer from Ukraine. He uses MTS Ukraine and says that when a SIM card is inserted, the phone freezes up.
Anyone know of a solution?
The iPhone is on iOS 4.1, has been downgraded to 5.13.04 baseband, and has ultrasn0w 1.1-1 installed.

Try reinstalling ultrasn0w (make sure 3G is disabled before doing this).. and maybe doing a hard reset while the SIM card is inserted.
If that doesn't work, a restore might be required.. via custom firmware, though, to keep the baseband.

Your post mentions iOS 4.1 and then downgrading baseband. Unless you used Tiny umbrella to upgrade to 4.1 while preserving the older baseband, I don't see how you could have downgraded bb 05.14.02. It's just not possible at this moment.
And there might be your problem. The phone might have locked up again...
