I've seen several tricks as the Yellowsn0w beta app in its current 0.91 beta version is slightly tricky for some to get their sim recognized.
[Edit Jan 3rd 2009: 0.94 and not the current release is 0.95, please update as the Dev team has fixed several issues checking out reports from users all over the world!

Here's the methods I've found on my personal quest the past few hours to unlock my sim the 2nd time.
My personal experience was at first a success after about 15 minutes when I popped in my girlfriends sim. it recognized hers and then mine when I swapped.
when My cousin passed by to unlock his own 3G I screwed my own 3G over and had problems activating it back.
after taking it easy (my cousin left after his was not done yet) I succeeded in unlocking my own sim again.
Here's the various methods I've seen in the meanwhile on several blogs and forums:
Method #1 (props to reda92 at youtube)

1. Install yellowsn0w thru cydia, keep your Third-Party Sim in the iphone
2. Reset Network Setting "Most important" 99% success rate will depend on this. (LF: you can find this in settings - general and to the bottom you'll see a "reset" menu in which you can find this)
3. Reboot your inphone
4. Wait a little over a min.
5. Remove your sim card and re-insert it back in
6. Wait 30 sec.
7. Turn on Airplane mode and wait 10 sec. "This is important as well"
8. Turn of Airplane mode, you should see "searching" start smiling T-Mobile carrier will start appearing.
9. Start make call!

a slightly different method for this worked for me the 2nd time.
differences were: I had everything like 3G, edge, wifi, and ssh off through sbsettings, I didn't pop out my sim, and at step 4 I didn't unlock the unlock screen but let it wait at it for 1-2 minutes

if you get "open: Recource busy" just repeat the command where you got it from.
if command yellowsn0w -q keeps loading for more than 20sec and your terminal looks similar to pics from Flickr: deli_gt's Photostream move to next step.
9. Go to setting turn airplane mode on wait 10 secs and it should start searching for the network, it shold find the network and everything should be ok.
If not
if it still show the four dots for the signal repeat the steps 8 and 9 but first exit mobile terminal totaly by holding down the home button until it closes totaly
if that doesnt work eather use
compination
go to terminal
type yellowsn0w -c
type yellowsn0w -r
type yellowsn0w -q
airplane mode on
airplane mode off .
That's how you get it working for 99% sure. i tried it in 3 diferent iphones one from finland one from germany and one from UK. UK and german wasnt workin in the first.
Until now many has goten it to work only by this method.
UPDATE.
I can confirm I got it allso to work with out sbsettings reset, just usual phone on and phone off.
UPDATE.
So only thing you have to actualy do is to install yellowsn0w from installer not from cydia. And after every time the phone boots wait 30 secs and make the commands with mobile terminal(step 8)-> and airplane on off, if doesnt work repeat the step 8 and again airplane on off.
UPDATE.
If you lose network use this compination and you'll get network back without reseting the phone
go to terminal
type yellowsn0w -c
type yellowsn0w -r
type yellowsn0w -q
airplane mode on
airplane mode off
