Default Sending my iphone in for repair HELP ME!

I need to send my iphone in for repair and it is currently jailbroken. What steps do I need to take in order to make sure that Apple does not know that my phone was jailbroken? Can I just update it to 2.0 and send it in or do I have to flash the baseband or bootloader oranything like that??? I have been looking around for a tutorial on this subject but haven't seen anything definitive.

Can someone that has sent their iphone in for repair tell me what they want shipped in the box? I do not have the sim card that came with the phone. Do they just want the iphone shipped back in the box?

I'd like to know other peoples experiences with having repairs done if they have been using jailbroken/unlocked iphones. My problem is that the vibrate is broken and rattles inside. I believe this is a hardware defect.

I do not know which method was used to unlocked the phone. I see that the modem firmware is 04.04.05_G but I don't know what that means to me.

Any help would be greatly appreciated!

If you are on 1.1.4 just update to 2.0 and it will wipe everything clean.
    Dirty as I wanna be!
