Forgot the name of a theme, Please Help

Hi guys, On my old iphone before this one, I had a theme that was basically horizontal bars that said the name of an app with a little picture. I remember having to use some blank white boxes to fill it in as well. I was wondering if anyone had any idea what the theme was called? It was about a year and a half ago roughly. It was really nice looking, Instead of each thing on your home screen being a little icon it turned each icon into a bar that went from left to right.

By the way its an iphone 3g, I also remember I had to create some blank white boxes with an app nd then drag like 4 boxes onto each line for the bar.

If you need any more info, Just ask and ill say what I remember, It was my all time favourite theme so bit annoyed I forgot the name
