Create the image in what? Photoshop? I don't have this software so I can't test it.

I finally got this to work. I used the transparent image that oldsystem provided and made copies and named them with the names that oldsystem provided and boom no pressed icon image

OK guys I've done some testing.
Bad news: If you want auto create icons on 4.2.1 WITH NO "pressed icon image" you CAN'T!!!
What I believe Crapple has done is fuse the "IconDarkeningOverlay.png" with the "AppIconMask.png". The "WHITE" area on the AppIconMask.png (ie the part of the icon that is NOT being masked) turns into the semi-transparent black "shape" (I say "shape" because it depends on what your AppIconMask.png is) that comes up when you press an icon.
If any finds a way around or would like to correct me feel free. But for me I believe it is a dead end as for now.

He All,
I have been going round and round in circles with this for about 3 hours now and i am pretty sure it's posible as i've done it before. I have customized the auto skinning icon for a theme and it looks good but the shadow stops one line of pixels short from the top of icons i have created but the auto skinning ones look fine. So when ever i select an icon i get a nice shadow across the icon then a bright line of colour at the top. Is there anyway to fix this without having to cut all the icons down by one line of pixels at the top?

I GOT IT TO WORK!! THE METHOD MENTIONED ABOVE IS CORRECT. A little unclear but it's correct. ** Thanks, oldsystem **
Do this and you'll be fine. No more Shadows when pressing your icons. As it turns out, it's take a "little" getting used to because you when press on any icon, you're used to seeing something then your app opens. But now that it's not there anymore, your mind think for a milli-second wether you've pressed it or not.
Steps:
1) Create a folder called 'com.apple.mobileicons.framework' in THE THEME FOLDER THAT YOU'RE USING. Put this new folder in a folder call Bundles. If there isn't one, create one and then put 'com.apple.mobileicons.framework' folder in it.
2) Get/make four blank icons named:
AppIconMask.png
AppIconOverlay.png
AppIconShadow.png
DefaultAppIcon.png
** if you're on iPhone 4, add @2x to the end of each file name ([email protected]) **

3) Once you got all your folder(s) created and your icons are named correctly, respring and you should see the changes.

Yeah this has become a nightmare. Originally I thought this worked, however after a reboot all my app store icons disappear leaving only a shadow in their place. Respringing is not an effective means of testing this.
I write Mac only tutorials for the iPhone and own every model. My iPhones run on the following SIMs/networks: AT&T, GoPhone, H2O, O2, SimpleMobile, T-Mobile and Verizon.
