ive been trying to figure this out too. I had some ideas that i was going to test (there is a conference framework i was going to try that has that facetime icon and its very similarly sized.)

if u want to give it a shot its [email protected] and its located in System/Library/PrivateFrameworks/Conference.framework. Let us know if your successful

I found a similar one in PhotoLibraryServices framework but it doesn't seem to be the same size.

I had a custom made (by me) iPod to iPhone tweak that made an iPod act like an iPhone. I somehow got the native phone app to tie into FaceTime flawlessly with no issues. I had no issues at all with it either. I honestly can't remember how to though, I could try giving it another shot.
