Anyone know what wallpaper this is and the name of the font? I can't remember!

It's a Cydia package called "Stack" and it's from the source "Ste Packaging".

Here's the wallpaper. Not sure about the font though. You can also download it off cydia by searching for the Minimal Lockscreen theme. There are 3 versions (dark, light, clear). Hope that helps.

Try just finding the .ttf file for the font and put it in "/var/mobile/Library/FontSwap/Packages/System Fonts/[Font Name].font" where [Font Name] is the name of the font (duh) like "Hand of Sean". Can't remember if I had to change the .ttf file but I've used this method to make my own fonts for FontSwap.

I don't think it's in Cydia. And you need to do change the font to work with the iphone. Here is a copy of the font that is all ready.

You need to put it in your font folder via SSH. Just replace the base font and you are fine.
