that is an awesome theme. I think it's a mix. I think the icons are realize icons. It's in Cydia and there are even more on the theme release thread here.

The dock is five icon dock flow with realize icons and a photo of an icon phone -- not sure exactly which dock flow, but you can search in Cydia.

That dock is awesome.

I've seen that wallpaper, but I don't remember where. I think someone else might have it. Lets see what people say.

I think that is a custom dock, I've never seen it before. It should not be that hard to make. I would ask someone either here or at macthemes2 to make it for you.
