I've never made a slider before but I'd assume Photoshop or something similar.

Slider icons are 47 pixels tall. The icon names should be bottombarknobgray.png, bottombarknobgreen.png, and bottombarknobred.png. These are used to unlock, answer calls, and power off, respectively. They should be stored in /Themes.<whatever>/<Your Theme Name>.theme/Bundles/com.apple.TelephonyUI/ The same icon can be used for all three, just make three copies of the file, and use the three different filenames above.

Attached is a sample Ohio State Buckeyes slider.
