I finally figured it out!!
Its almost the exact same method as adding a background!
What you do is edit my PSD (located below) in photoshop, and replace my keypad with the one you want, BUT you must not move it from its current location.
Save it as BarDialer_Sel.png
Then copy it to /Applications/MobilePhone.app/
and your good to go!
The only drawback is that theres no keypressed action, but it still works perfectly!
Heres the .PSD, and just incase you dont have photoshop i included the .png file
Heres a template of the keypad i used, simply copy the .ttf font file into your "cWINDOWS\Fonts\" directory and then open the psd with photoshop and slip your own image underneath the numbers.

Now we need to find out where the pressed button resides, and then Wiki this up!

You can do the voicemail the same way. I borrowed your idea and here's my copycat upload. ^_^
This is a blank .PSD file that people can edit to add the image that they want. I didn't add any graphics to it.
Only problem with this is that if you tap the more button (>) to go to contacts, the picture cuts off some stuff.

Great job on this new hack! I don't mean to sound inconsiderate but will there be different pictures to be put on the keypad eventually?

You can use photoshop to put your own image on the dialer pad. The pictures that are uploaded are more of a guide for *where* to place the images. The selected icon has to be in dead center of the image to make things line up right.
OH! And I found a nice trick. I opened the .psd file (either mine or Jonny's), set the image that I wanted, merged all the photoshop layers, then set the layer opacity to 80% and now I have a working keypad with an image over it that hilights when you press a button. ^_^ Very spiffy.

Don't be afraid to drop that opacity down to 50%. You'll pick up a lot more of the blue button when pressing numbers this way. The main picture still looks pretty good at this level.
I'm in the process of making a super clean template to use for this. Everything will be on its own layer. All you do is drop in your image, the numbers, letters and gray and black tool bars will be there too.
