I made it myself, but I can zip all the files together and upload them. I'll give you a quick rundown of what I used to get it like that.

-vWallpapers:
Downloaded the free app, and I made my own wallpaper. The ones provided aren't very good, in terms of video quality.

if you want to make your own video wallpaper. edit a video file to the resolution of 320x480 and encode it to a mpeg-4 file, then copy to this location:

/var/mobile/media/skrew/videos/




-iBlank:
created a single blank icon for the main page. Without any icons, the page disappears, and pg2 would become the mainpage.

-infinidock:
changed settings to 9 icons and scrolling/paging/scroll snap are all turned OFF

Then I made the icons and created my own .theme folder and ssh'd them into my phone.
