Delete device support file&kill itunes helper
    Create a new custom fw
    Put it into dfu mode
If you get error, close itunes&re-open it
    Restore

Forget itunes helper.1st be sure put it into dfu mode properly. It will be just blank page.Go to itunes...your user name/library/itunes/device support. If there is a file in there delete it.Then follow that guide...
