advice please: detecting appointment alert

Hi

I really need a more persistent appointment reminder system than the "1 alert better hope you hear it" system that the iphone has currently.

I would like to modify the sms alert script to do this, but I can't find out how I can detect that an alert has triggered. there seems to be no change in the calendar database.

can someone more knowledgable than me please point me in the right direction? Basically, if there's a message on screen saying "appointment in 15 mins etc" I want to be able to detect that.
