My iPhone turned into iPod Emergency really need your HELP : ( with IPHONE

I got a problem today that i was installed software i believed it's call map that can view offline. After I power off my iphone has ipod name on the left corner (usually that place should be T-Mobile and have connection bar) middle: time and right hand side have battery image. If I used my bluetooth headset I am able to make a call but when I click on the Home button it's just blank with a picture background. Is there anyway that you can set it back to iphone with menu function that you can click on. Really appreciate your help.

P.S. I am able to use bluetooth to call out

try to unistall the app through SSH but most likely a restore should fix it. good luck=]
