Install CyDelete and go into the Settings app and make sure "Delete non-Cydia apps" is on. You'll be able to delete Freeze then.

So again, if anyone else is having the issue of Freeze not showing up after reboot, try restoring then jailbreaking BEFORE adding all your data back onto the phone.

Before you tried to re-jailbreak it, did you have Freeze hidden with SBSettings? If you did, then that is why it wouldn't show on the springboard.
