An access point (APN) is an entry point into their network, where they can monitor the amount of bandwidth you use. At the moment, we are using the same APN for tethering and for mobile browsing, but pretty soon they will have a seperate APN for tethering specifically, so they can monitor it seperately and charge you different rates (presumably).
Think of it as having 2 seperate internet service providers at your house. One you use for sending the occasional email, and the other you use for heavy downloading. In this example, they would be able to monitor your email usage, and you download usage seperately, and charge you different rates on each account. If however, you forced your heavy downloads through your 'light usage' account, they would see an increase in the amount of data being used on that link. So they would be able to tell your usage had gone up, but I don't think they can poll your device to see how you did it.
What they can, and probably will do, once they have the tethering APN active, is send your phone instructions, to use the new APN for tethering, so that they can charge you specifically for tethering and not for other data usage.
To answer another question, I am using Windows Vista with USB tethering. I haven't tried bluetooth yet.
