Yeah I did that too, about a week after I got my iphone.
But I used black, shiny carton and a larger base ramping up slightly, right up to the bottom of the display on the front side, so my screen has a nice reflection below it, very similar to the coverflow thing in the iPhone's iPod.2007 meets 1982: iPhone + akaRemote > (lotsa stuff >) > TB-303: Controlling the Bassline with the Accelerometer

K, don't know if anybody else has actually tried, but its bit of a ***** to actually do. The front bit confuses the hell out of me, not sure what goes where, tried a few different ways, anybody know?

I finally put it together when I found out that it's too small for my iPhone. I'm not sure whether the printer resized it or if the picture was too small... Luckily the phone still fits, but I'm not sure how long it will survive all stretched out.
Recommendation: Use card, not paper. In fact, the card only just came out in one piece with all the folding I had to do.
Oh and one other observation: The piece of paper in the picture has the logo in a different place to the one that you download. So that might be slightly confusing.
