Here's how I do it, I'm not very experienced so there might be someone who can explain a better more efficient way to complete the process. I am using CS5 but guessing CS6 is mostly the same. These instructions are after you have added a layer with the icon you want to mask and have adjusted it's placement accordingly. I hope the steps make at least some sort of sense. Good luck.
