For the Action Menu --> Raw File System --> Library --> you will see the Action Menu folder, just drop that I made in there. rememeber to back up the old one.

The size is exact same as all other icons make sure you click on the image i upload and dont save straight from my post. Think i got rid of the glass image let me know
