Default iPhone 4 vibrator stopped working.. very weird symptoms also
    Hello
well, the vibrator stopped working after it worked fine on my iPhone 4 GSM IOS 6.1
    I tested several things:
    1. run the iBrate app did not fix the issue
    2. i disassemble the vibrator and checked it with external power supply unit and it is working and vibrating
    3. i check the contacts under the vibrator with a Multimeter and got about 80mV - so the vibrator is getting voltage.

    what could be the problem?

    Regards,
    Mosh

Make sure the tape around the headphone jack hasn't lifted. If it has then that could be blocking the motor from spinning.

It could be the contacts aren't lining up on the headphone jack or you may need to replace either the motor or the audio flex cable (which is where the contacts for the vibrating motor are located). Those would be my next 2 things to check. Since you're getting some voltage to the contacts it's probably the motor itself so check that first. Cheap part.
