Restore to custom firmware of 4.1 and you will be able to unlock too.
This is what you do :
Update your itunes to the latest.
Download Redsn0w 0.9.6b2, 3 or 4 is fine, download the stock 4.1 ipsw.
Now run redsn0w and browse to stock 4.1.
Check only the box : DFU mode only.
Perform this action until you see a message DFU done.
Now open up Itunes and shift + restore to this custom firmware of 4.1 which is in your PM box.
You can install Ultrsn0w to unlock to use with you current carrier.

Just close tiny umbrella, put the iphone into dfu then try restoring it. If you require unlock you should follow Yeow's instructions though.

I am in a similar predicament and I was wondering once we restore the iPhone, would the SIM for the original carrier be required to activate the phone or not?

The important part is Geohot's DFU , you must get it right and then shift + restore to my link firmware.
Close tiny umbrellla, you don't need it.
If you follow closely what i said, you will restore successfully.
