Icon and sound problem since jailbreak

I have a jailbroken 3gs

the icons on some apps change to the wrong icon at random ie 1 or 2 of them

also twice now i have lost sound phone and headphone , ie similar problem to the 2g issue a long time ago

anyone have any solutions for either of these issues ?

This post is a little old but I'm having the same problem. Here's a couple links that I've found to fix it. The problem for me though is that the fix is just for apps that are downloaded through the apple store, and not through Cydia.
