Verizon iPhone 4 iOS 5.0.1 Speakers not working?

Hi there,

I got my girlfriend an iPhone 4 for Christmas, good boyfriend I know, however, just recently it started acting up and won't play music through the built in speakers or through headphones, instead it shows the following screen when in the music app:

In addition, her ringtone will not play, nor is she able to adjust any volume settings using the side buttons, I've gone through her settings on the Settings App, Springtomize 2, as well as removing suspicious packages from Cydia, however it still does this, any suggestions guys?

I have tried to do a fresh restore, but I keep getting an Error Code, I will edit this post with the Error Code tomorrow when I get it from her.

bring it back to apple store and get a replacement.

yup, electronics do break, wear and tear........even Apple products
