I'm sorry, but if you're consistently getting error 40 no matter what you do or which way you do it, it looks to be a hardware failure which software methods won't fix.

Well I know you want to keep your unlock but I would prefer to use stock firmware for this situation. Just try to restore to a stock 4.0.1, your unlock won't be affected because 4.0.1's baseband is unlockable. If it were me, I'd be avoiding custom firmwares just for right now. So run TSS server and shift+restore to stock 4.0.1.

It wouldn't. It's pretty obvious it's a hardware failure. But if it were you wouldn't you exhaust all options you could try? I already figured it's not gonna work with any software method, but understanding the situation if it were my phone I wouldn't believe it until I have tried it all.

Never say die ? Lol.. If it were mine I would have got frustrated and threw it in anger.. I threw my iPhone 2g away from 11th floor coz it stopped working and was not working after repeated tries ofcourse I had to face some pretty hard firing from mum dad that night lol ..
