SMS mixing up

i have searched the forums for an answer to my dilemma and if i have overlooked a solution i apologize in advance for asking a question which was already answered.

occasionally ill get a sms and on the preview it will show the message and say attachment 1 image. the sender did not send an image and then when i go to view the text the message is gone and or there is a deifferent message from a different person all together. also ill send a pic of something and it will send a different pic then what i selected.

any info on a fix for this, what steps may i take to find a resolution?
