How to remove the untitled icon

need some help here , do anyone know how to remove the untitled icon ?

it will enter safari when i click on it.

Huh, odd. may be a web clip, are you using a theme that auto creates icons?

i'm using buuf theme, i try removing it like other app but it came back after i restart my phone .... did anyone got any idea wat can i do ?

DO you know how to SSH into your phone? You navigate to
Private/var/mobile/library/webclips and delete any clips there.

yep i know how to use SSH .... i will go back and time tonight ..... i think this may helps ....thanks
