2.2.1 and the headphone remote issue

Ever since 2.2.1 I have been getting requests to repair headphone jacks that the remote has stopped working on. As I have told many, it is not the jack..... It's software. Many do not have the issue, so we need to trace down the cause of why the headphone remote is not working for many in 2.2.1 so we have a reference thread to point them to for help.

If anyone has reason they have found for the headphone cable's remote not working in 2.2.1 please add to this thread. Thanks!
