They did six (6) for me. After my original defective phones, they just kept throwing refurbs at me, so I eventually said shove it to my local Apple retail store and called Apple's iPhone line. If you can ***** enough and pull the "longtime Apple customer, but reconsidering" card well enough, you'll be able to get hold of a "Customer Care Administrator". The guy I got was amazing, I somehow had the bad luck of two MORE defective iPhones, but he didn't sweat it. Kept shipping phones in and out until I got a good one.

This is my first Apple product, so I wasn't even aware you could call them direct. But wouldn't they just refer you down?
