I just got mine in this morning, earlier than expected. Big box for such a small product. I filled out the form as soon as Apple made the announcement on the recall. Also, when shipping back the original product, you have to use DHL, not FedEx, which sucks cuz the closest one is 30 minutes away. You can always call for a pickup though.

And I'm just curious, what happens if I don't send the old one back? I'll be damned if I'm going to go out of my way to return something to them that they should have gotten right to begin with. They don't have my CC on file so it's not like they can charge me. I'm sure the tin foil hat guys would say "but but but they have your iPhone serial and they will shut your phone down omfg lolz oh noes!"

Well it says you have 10 business days to return the original. Failure to do so, Apple has the right to charge your card of a non-return fee of $29. I'm wondering how they can since I delete my card information from the iTunes store after every use.... and they better not have that information backuped somewhere. I would've actually liked to have kept the original, I didn't even use it, nor do I plan on using the new one, oh well.

I called DHL (# in invoice) and they picked up the package within an hour.

This is the nature of recalls guys. It isn't a method to get free product. Of course you should have to return the recalled item to get a new one...
