Have you set the permissions of the application to 0755?
I had this problem and once I changed that it worked.

You need to change the name to Anysim.app for it to work.

as said above, incorrect naming is the most likely cause.. happened on mine before
the folder needs to be exactly "anySIM.app" (capitalisation matters), inside the folder should be the various files (no subfolder).
also check that the permission of the "anySIM" file inside that folder is 0755.
check that BSD Subsystem is installed, and you're inserting a NON-AT&T SIM when running anySIM
lastly, check that Auto-Lock is set to Never

You need to go back to the baseband of 1.1.2 (04.02.XXX).
anySIM 1.2 can only unlock the 1.1.2 baseband.
This guide might me interesting for you:

He's trying to run anySIM 1.2 on the 1.1.1 baseband, that's not possible.
The information is correct!
He'll get the 04.02XXX baseband by updating to 1.1.2 and after that downgrade to 1.1.1. AnySIM will run fine in that case.
During the downgrade to 1.1.1 the 04.02xxx baseband stays intact.
anySIM 1.2 is only meant to unlock the 04.02 baseband, it says that clearly when you start it up.iFoneguide

Hi,
I have the same issue yet I have upgraded to 1.1.2 and also have the correct baseband 4.02.13.
Each time process goes smoothly untill the anyApp1.2 install then it starts and just freezes no errors.
Please dont direct me to a bogus site
all permissions and filenaming is correct as above.
Any ideas........help (mine was initially 1.0.2 which I unlocked then revirginized and upped to 1.1.1 and unlocked succesfully also upgraded basedband)
