My default HOSTS file on Windows Vista consists of the following:

    # Copyright (c) 1993-2006 Microsoft Corp.
    #
    # This is a sample HOSTS file used by Microsoft TCP/IP for Windows.
    #
    # This file contains the mappings of IP addresses to host names. Each
    # entry should be kept on an individual line. The IP address should
    # be placed in the first column followed by the corresponding host name.
    # The IP address and the host name should be separated by at least one
    # space.
    #
    # Additionally, comments (such as these) may be inserted on individual
    # lines or following the machine name denoted by a '#' symbol.
    #
    # For example:
    #
    # 102.54.94.97 rhino.acme.com # source server
    # 38.25.63.10 x.acme.com # x client host

    127.0.0.1 localhost
    ::1 localhost
So really it's just the instructions on proper use of the file and the default ones. If you want to add the "74.208.10.249 gs.apple.com" to the bottom, you can. That will redirect any request for Apple's servers to Saurik's, which in most cases will simply pass the request onto Apple.

Even with a blank HOSTS file, you should be able to restore to the latest firmware. Or are you trying to restore to your current 4.0.1 firmware? If that's the case, then I'd try using TinyUmbrella to pull your SHSH files from Saurik's server, then start the TinySS server. Then shift+click Restore in iTunes to select a downloaded 4.0.1 ISPW firmware file, and the TinySS server will approve the restore request using your SHSH file even though Apple isn't signing that firmware anymore.
