its a apple universal dock..i forgot what case the adaptor came with i just did a little modding to get it to work with the case..i just put a skull carbon decal i had left over from other projects came out pretty good remote works/usb/earphone out..it match really well together

i remember what dock adaptor it was from a agent18 for a iphone 1st gen but i forgot what case model it was..

as for decal..
Parma Carbon Skulls Decal Sheet 8.5"x11"

its really easy to slap a decal on a apple universal dock because of its simple design just need a sharp new blade..
