are you kidding? Check almost anywhere on the site. You should already have cydia or installer on your phone. Use them or ssh or any of the Wiki guides at the top of the page.

how do you manually install one using ssh?

if i have the *.app file, where do i put that to make it work?

well first of all you have to have OpenSSH installed on your phone via installer or cydia. Then use an SSH client then just drop it in your application folder of your phone. check the wikis Main Page - ModMyI - Wiki
