You will need to use redsnow to reinstall the iPad baseband again too. If you updated to regular 5.1.1 when you had the iPad baseband installed it puts your baseband in an inconsistent state. So a reflash is needed first.

If you're already on ipad Baseband, use redsn0w_win_0.9.14b2 to downgrade. Select ipsw iPhone2,1_4.1_8B117_Restore.ipsw..... After that, use sn0wbreeze-2.9.5 and make a custom firmware using iPhone2,1_5.1.1_9B206_Restore.ipsw. Following on-screen instructions in snowbreeze. When upgrade to 5.1.1 is complete, you should have 5.1.1 on ipad baseband. Now go back and use same redsnow, put phone in DFU mode.... Tick Downgrade iPad Baseband.... When all is said and done, disable 3g & cellular data on phone. Open cydia and install ultrasnow. Wifi is required! Insert sim and give me a call....
Now was it that hard? Maybe the 3gs guru's in here might be of little more help on this....

+1. The downgrade of the bb should resolve it. Sometimes some people on the iPad bb still can't get ultrasn0w to work with it.
Although the above instructions seem a bit complicated...
If the device is already on 6.15.00 and on 5.1.1, simply run redsn0w, hit jailbreak, set device into DFU, select downgrade from iPad bb only, then you're good to go.
If you are not at 5.1.1 in a functional state, simply use redsn0w and in extras select create custom ipsw. Do so with a stock 5.1.1 firmware file. Then use iTunes to shift+restore to the no baseband custom 5.1.1 ipsw. Then do the steps above with the baseband downgrade.

Only if he upgraded to 5.1.1 with custom firmware using snowbreeze. Snow breeze does activate the phone during the process. If upgrade with redsnow and phone requires you to activate with original sim, you might get some kind of problem with your signal. I always downgrade to 4.1 w/ipad baseband and work my way up using the steps I posted above. So far, I haven't had any issues....
